ITEM DELIVERY: if your donation is a tangible item and you wish to drop it off at the ElderHelp offices, please call us first at 619-284-9281 then visit us Monday-Thursday 8 am to 4 pm at 5095 Murphy Canyon Rd. Suite 100, San Diego. To make arrangements for pick up, please email auction@elderhelpofsandiego.org.

NOTE: this form does not serve as a receipt for this contribution, but is intended for our internal record keeping purposes only. A receipt describing the items or services donated will be mailed to the address supplied. ElderHelp of San Diego is unable to include the estimated value on the donor receipt.
