Care Coordination
Care Coordination creates opportunities for individuals to live independently and thrive in their own homes.
ElderHelp Care Coordination clients receive personal visits in their homes with Care Coordinators to create a customized network of support tailored especially for them. We connect them to community resources and help them navigate health and wellness challenges that can seem overwhelming without the guidance of a trusted advocate.
Clients are matched with vetted volunteers, genuinely dedicated to serving older adults and providing services such as:
- Grocery shopping (either for or with the client)
- Organizing mail and paperwork
- Social outings to the beach, parks, concerts or movies
- Friendly visits to play cards, games or just chat
ElderHelp’s Home Safety & Maintenance service is available through the Care Coordination Program, or for community members in need of a one-time project. Volunteers complete home safety assessments, minor safety related home repairs, replace smoke detectors and lightbulbs, and install safety bars. Another service through the Care Coordination Program is ElderHelp’s Food Pantry. Our Food Pantry deliveries provide essential supplementary nutrition to seniors across San Diego County. Clients enrolled in the service receive a monthly call to confirm their need before each delivery. The deliveries typically include fresh foods such as milk, eggs, dairy, meat, cheese, and a variety of fruits and vegetables when available. These are complemented by regular dry staples like oats, pasta, beans, and rice, along with some non-perishables. Additionally, other vital supplies, such as adult incontinence products, hand soap, shampoo, face masks, and hand sanitizer, are available upon request.
Most of our clients live alone and rely on ElderHelp’s vital services to stay healthy and connected to the community. Reducing isolation and the risk of falls in the home through a network of support promotes greater well-being for the client and eases the burden on our overloaded healthcare system.
ELIGIBILITY
To be eligible for Care Coordination:
- You must be 60 or older
- Reside in our service area (see below)
- You must be able to advocate for yourself
SELF ADVOCACY
ElderHelp’s programs are structured to help older adults maintain their independence, therefore it is essential that our clients are able to communicate directly with us to complete assessments and maintain a schedule. The person who would be receiving services directly, must make the initial call to begin the process and manage their own appointments and schedules. While it may seem easier to set up services for a loved one, the program will not be successful unless the individual who will be receiving services is interested, informed, and engaged. ElderHelp does recognize the importance of a good support system and how valuable family is to the team.
